To provide clinical and wellness services, we may collect:

  • Personal Identifiers: Name, date of birth, mailing address, email address, phone number

  • Protected Health Information (PHI): Medical history, health goals, laboratory results, and clinical consultation notes

  • Payment Information: Billing address and payment method details

Credit card information is processed through secure, PCI-compliant third-party payment processors and is not stored by the practice.

HOW WE USE YOUR INFORMATION

We use PHI for Treatment, Payment, and Healthcare Operations, including:

Treatment

To perform clinical evaluations, develop personalized metabolic or weight management plans, manage medications, and coordinate care with laboratories, pharmacies, and other providers involved in your care.

Payment

To process payment for services. If you elect to use third-party financing (such as CareCredit), we share the minimum necessary transaction information to facilitate financing.

Healthcare Operations

To improve services, conduct internal audits, ensure compliance, manage administrative functions, and provide appointment reminders or practice communications.

DISCLOSURES TO THIRD PARTIES

We do not sell or rent your personal information.

We may share PHI when necessary for:

  • Pharmacy partners to fulfill provider-directed prescriptions

  • Laboratory partners for diagnostic testing

  • Financing providers if you choose a payment plan

  • Legal or public health requirements, including court orders or mandatory reporting

All disclosures are limited to the minimum necessary as required by law.

Request access to or copies of your medical records. We may charge reasonable, cost-based fees as permitted by applicable state law, which may vary depending on the state in which you reside. You will be informed of any applicable fees before records are released.

Request corrections to information you believe is inaccurate or incomplete. We may deny the request but will provide a written explanation.

Accounting of Disclosures

Request a list of certain disclosures made outside of treatment, payment, or healthcare operations.

Request a paper copy of this Notice at any time, even if you received it electronically.

TELEHEALTH & ELECTRONIC COMMUNICATIONS

We use HIPAA-compliant platforms for telehealth and electronic communications. While safeguards are in place, no system is completely risk-free.

Electronic communications are not monitored continuously and must not be used for emergencies.

USES REQUIRING YOUR AUTHORIZATION

We will obtain your written authorization before using or disclosing PHI for purposes not described in this Notice, including marketing. You may revoke authorization in writing at any time, except where action has already been taken.
